Bouffant caps and surgical caps both cover hair. But they are not interchangeable. A bouffant cap is a round elasticated cover that looks like a shower cap. A surgical cap ties or snaps at the back and fits close to the head. Choosing the wrong one means either hair escapes, staff complain about comfort, or you violate OR protocol.
Here is the comparison from a manufacturer who supplies both to hospitals and food plants.
The Short Answer
Bouffant cap: Round, elasticated, easy to don, covers all hair. Best for food plants, cleanrooms, general medical areas.
Surgical cap: Ties or snaps, fits snugly. Best for the OR where a bouffant may interfere with the hood or mask.
Both meet hair containment requirements. The choice depends on the environment.

Bouffant Caps:
Bouffant caps are the round, puffy elasticated caps you see in food plants and visitor stations. They are made from non-woven polypropylene and come in 21", 24", and 26" diameters.
Pros:
- Fast to don - slip on and go
- Covers all hair regardless of length or thickness
- Roomy - comfortable for long shifts
- Low cost
- One size fits most (with diameter options)
Cons:
- Can gap under a surgical hood (the puffy shape doesn't sit flat)
- Bulky under some hoods
- Less secure if the elastic band is thin
Best for:
- Food processing plants
- Cleanroom gowning rooms
- Hospital visitor stations
- General ward and outpatient areas
- Pharmaceutical manufacturing
Surgical Caps
Surgical caps are the close-fitting caps worn in operating rooms. They come in two styles:
Tie-back surgical cap - flat cap with two fabric ties at the back. Ties are fastened securely.
Snap-back surgical cap - elasticized back with snaps for adjustment.
Both are made from non-woven PP or SMS fabric and fit close to the head.
Pros:
- Fits snugly under a surgical hood
- No gap at the nape
- Secure - won't shift during a long procedure
- Professional appearance
Cons:
- Slower to don (must tie or snap)
- Less roomy - uncomfortable for very thick or long hair
- More expensive than bouffant
- One size may not fit all head sizes
Best for:
- Operating rooms
- Surgical suites
- Dental surgery
- Anywhere a hood is worn over the cap
What About Beard Covers?
For staff with facial hair, neither a bouffant nor a surgical cap covers the beard. You need a separate beard cover - a small elasticated mesh or non-woven cover that fits over the chin and cheeks.
In the OR, a beard cover is worn under the surgical cap. In food plants, it's worn under the bouffant. We manufacture both.
Common Buying Mistakes
1. Ordering one size for everyone. A 21" cap won't fit a man with thick hair. Order 24" as default and stock 26" for larger heads.
2. Buying 10 gsm for medical use. 10 gsm is too thin for OR or food standard. It tears easily. Use 15 gsm minimum.
3. Forgetting the beard cover. Staff with facial hair need a beard cover in addition to the cap. Don't make them choose.
4. Using bouffant under a surgical hood. The puffy shape gaps. Use a tie-back surgical cap in the OR.
5. Checking the elastic. Thin elastic (2 mm) breaks. Look for double-stitched elastic at least 5 mm wide.

FAQ:
Q: Can I use a bouffant cap in the operating room?
A: It depends on your facility's protocol. Many hospitals require tie-back surgical caps in the OR because they fit under the hood. Bouffant caps are commonly used outside the OR.
Q: What size bouffant cap fits a man?
A: Most men fit a 24" cap. For larger heads or thick/curly hair, order 26".
Q: Do bouffant caps cover long hair?
A: Yes, if you order the right size. 26" is designed for long or thick hair.
Q: Are bouffant caps reusable?
A: No. They are single-use disposable items. Reusing them defeats the hair containment purpose.
Q: Do you offer beard covers?
A: Yes. Beard covers are available and should be used alongside bouffant or surgical caps for staff with facial hair.
